My mother, who walked regularly before moving in, was kept in a wheelchair all day and even lifted by her pants seat when taken to the bathroom—causing her distress. I frequently found her in soiled diapers, and her personal cleanliness and comfort were neglected. The activity area was unbearably hot—often near 90–100 degrees—and residents were mostly left sitting around with the TV as their only stimulation. My mother also developed a bruise on her arm while staying there, and when I expressed concern and asked staff to handle her more gently, I was met with defensiveness rather than compassion. The facility appeared unclean—on one visit, I witnessed a large cockroach crawl into my mother’s room and nearly onto her. Days later, another dead roach and even dried human feces were found under her bed. To make matters worse, I noticed my mother wearing someone else’s bra and missing her prescribed dementia medication patch.

My mother, who is in the late stage of Alzheimer’s disease, was moved into The Phoenix Memory Care Unit at Willow Towers Assisted Living in December, 2025. Unfortunately, my experience there was deeply distressing and alarming from the very first day, and I moved her out within a month. The day after my mother moved in, I witnessed multiple residents yelling in distress. One male resident became upset and used inappropriate language, to which a care aide responded by screaming at him, also using profanity, and repeatedly yelling “SIT DOWN!” This outburst was extremely upsetting, and no other staff intervened to calm the situation or support the resident. When I asked the Care Manager if this behavior was normal, she apologetically said, “Yes.” I knew then that I had made a mistake in bringing my mother there. Throughout the next two weeks, my concerns only grew. The environment on the third floor (The Phoenix unit) was consistently chaotic. The staff rarely smiled or interacted warmly
